Description

Site Visit for a tour of the facilities and administrative space and discussed current programs, including the HIV management program. Described the new HOME shelter and health clinic and the pathway to referral to this shelter. Reviewed SUD programs and education. Provided information and referral form for Action Wellness Perinatal Case Management services for pregnant and postpartum women with HIV and chronic illness, including SUD.
