Course Description:
Ryan White Part B Case Management Standards teaches Ryan White-funded case managers hands-on instruction to help them learn the policies, processes, and practice of Ryan White case management in the Commonwealth of Virginia. This program teaches case managers all steps of intake, assessment, service planning, evaluation, peer review, and technical assistance.
This program will provide an overview of the 2023 Updates to the Virginia Ryan White Case Management Standards.
Objectives:
By the end of this course, participants will be able to:
- Relate a general definition of HIV/AIDS Medical & Non-Medical CM
- Describe the shift from general case management to a medical/clinical case management model.
- Demonstrate Screening, Intake, and Assessment procedures.
- Develop an Individual Service Plan.
- Demonstrate knowledge of appropriate indications for reassessment and discharge.
- Identify resources for QA Technical assistance.
