This Community of Practice (CoP) is a regularly occurring meeting for Delaware all professionals especially healthcare/behavioral health members working with populations affected by syndemics and HIV. We will come together regularly to learn more tools for our toolbox to support communities, become a trusted messenger, empower clients to engage in their health, and reduce syndemics like HIV, STI's, viral hepatitis, SUD, and mental health. 

Objectives:

• Share best practices for engaging or re-engaging clients in care to have a positive effect on HIV infection and reduce syndemics.
• Generate new knowledge to reduce the burden of syndemics like HIV, STI's hepatitis, SUD and mental health. 
• Discuss case-based scenarios to facilitate interactive learning
• Highlight challenges and successes to foster a culture of continuous improvement
• Understand the interconnectedness of syndemics and how to address multiple conditions simultaneously.
